About Primary Care Coalition

The Primary Care Coalition (PCC), founded in 1993, is a private, non-profit, charitable organization, serving as an advocate for Montgomery County, Maryland residents who lack health insurance or the resources to pay for healthcare. PCC in partnership with Community HealthLink, a coalition of ten community based safety-net clinics, provides high quality, effective, safe and efficient care for low income, uninsured predominately racial and ethnic minorities. Since 2005, the PCC has also administered Montgomery Cares, a public/private partnership that is tasked with providing primary and preventive health care to 40,000 low-income, uninsured county adult residents by the year 2010.

Challenge

PCC needed an affordable, user-friendly product to create ID cards for participating patients. The software needed to automatically modify the title and layout of the ID card for each corresponding clinic or program. Additionally, PCC needed to encode magnetic stripes to link each ID card to the patient's electronic medical record. To reduce costs, the PCC was looking for a software that could merge their two existing ID card programs and batch print ID cards for multiple programs.

Solution

Linking ID Flow to their electronic medical record program, the PCC is able to create a single ID card template with customized layouts for each clinic or program, saving much-needed time and resources. ID Flow's magnetic stripe encoding capability enables the PCC to imbed essential patient information in each card. This allows for quick and accurate access to the patient's electronic medical record at the clinic.

The PCC is using ID Flow's barcode capability to seamlessly merge with a medication access program. They are able to quickly imbed a medication access barcode compatible with the strict requirements of local pharmacies, allowing patients easier access to their prescriptions.

With ID Flow, the PCC has been able to launch their patient ID card program, ensuring expedited access to healthcare for Montgomery County's 40,000 uninsured adult residents.
